The Food and Drug Administration found the transfusion last Dec. 21 contributed to the patient's death. Center donor director David Graham said the death stemmed from "a very rare series of mistakes." Citing patient confidentiality, Graham said he could not discuss the hospital or patient. He said the blood center discovered the problem and notified the hospital within hours, but the platelets already were used. A March 9 letter from the FDA said the center's procedures are "not always maintained and followed." The agency also cited inadequate training and said the center didn't maintain adequate records of donors who had reactions.
